In the recent past, several papers have pointed to the possibility that tumour removal generates a permissive environment in which tumour growth is potentiated. This phenomenon has been coined “perioperative tumour growth” and whilst it represents a departure in terms of our attitude to the surgical process, this concept was first hinted at by Paget1Sir James Paget (1814–1899) was a surgeon and physiologist who is widely held (along with Rudolph Virchow) to be the father of the science of pathology. Despite this, the time interval immediately before and after cancer surgery (i.e. the perioperative period) remains an underutilised interval during which chemotherapeutic regimens are rarely implemented. Herein, we present a summarised review of the literature that supports the concept that tumour removal may potentiate the growth of residual neoplastic disease. We also outline current knowledge regarding underlying mechanisms and in this manner highlight potential therapeutic entry points. Finally, we emphasise the urgent need for trials of agents that could protect patients against the harmful host–tumour interactions that may occur during the perioperative period. Upshot: Borg et al. provide a framework that contributes to a growing body of research on how radical constructivism can help teachers and researchers to understand the complexity of classroom interactions. The bulk of my commentary is written to clarify theoretical points that I think are important to the (...) endeavor that Borg et al. set out for themselves. The sudden emergence of the COVID-19 pandemic brought global disruption to every aspect of society including healthcare, supply chain, the economy, and social interaction. Among the many emergent considerations were the safety and public health of the public, patients, essential workers, and healthcare professionals. In certain locations, clinical research was halted—or terminated—in deference to the immediate needs of patient care, and clinical trials focusing on the treatment and prevention of coronavirus infection were prioritized over studies focusing on other diseases. Difficult (...) decisions were made rapidly; flexibility and reconsideration were necessary not only because the intensity and severity of infection varied over time and by locale but also because knowledge of the disease and understanding of its treatment grew. Here we discuss the ethical challenges in decision-making and competing ethical tensions during the pandemic in an effort to advance future preparedness. (shrink)

Cifarelli & Volkan Sevim. Upshot: Cifarelli & Sevim outline the distinction between “representation” and “re-presentation” in von Glasersfeld’s thinking. After making this distinction, they identify how a student’s problem solving activity initially involved recognition, then re-presentation, and finally reflective abstraction. THE PASSAGE CITED IN THE TITLE IS commonly said to deal with puzzles or paradoxes about size and other measurable attributes of bodies. Nearly all recent commentators seek to interpret this portion of the dialogue as supporting or otherwise cohering with the Protagorean position Socrates expounds in the Theaetetus. On the present analysis, however, the support or harmony is mere appearance. The puzzles Socrates brings up are indeed associated with entities rejected by Protagoras. Socrates certainly uses the puzzles to foster (...) Theaetetus's trust in Protagorean doctrine. All the same, Socrates' procedure should not be taken at face value. The passage, which is an important digression, is fundamentally quite critical of the Protagorean position. (shrink)

In July 1993, the Scottish Highlands Centre for Human Caring sponsored a conference with the title Exploring the Spirituality in Caring. The papers given at the conference and included in this volume are offered as a contribution to the debate that must take place in nursing and in the wider context of health care provision. Ann Bradshaw's paper puts the debate in context arguing that nursing is fundamentally a loving response to the human being created in the image of God.
